Our office is gathering information from residents who filed written Notices of Claim concerning these flooding events. The Initial Response Form allows you to confirm your contact information, identify the date (s) of flooding that affected your home, and request additional information regarding the investigation and a potential claim.

After you submit the Initial Response Form, the confirmation email will provide a link to download the separate Flood Survey Form.
